I had the same problem with my TDR, it turned out to be a bit of corrosion on the hammer guide. As above, it's usually down to a number of reasons.

Setting the stages up was clearly written out on the old AAOC forum. I know how to do it but will need some time to write it down as I physically go through it myself. Will try and do it by next weekend as I have a mad week ahead.
